That is the case of a 6-month-old boy (living in Binh Hung Hoa, Binh Tan), with COVID-19 suffering from laryngeal edema, obstructing the airway, and acute respiratory failure. The baby was admitted to the hospital with shortness of breath, cyanosis, chest tightness, SpO2 (capillary blood oxygen concentration) decreased to 82%. Two days before, the baby had a low-grade fever, runny nose, cough and hoarse cry, wheezing. Her mother also recently contracted COVID-19.

Doctors gave the baby oxygen, anti-inflammatory, and adrenaline aerosol sprays to combat airway edema. After an hour of intervention, the baby is still having difficulty breathing, so a combination of two types of aerosol adrenaline and budesonide should be used. After 5 days of intensive treatment, the baby’s health has improved, no need for oxygen support, can eat and drink, continue to monitor and treat COVID-19.

According to Dr. Tien, the acute laryngitis to the extent of respiratory failure in this child is the most severe complication of respiratory failure, at level 2B; other children with respiratory failure grade 1 and 2A, recovered earlier. This is a very rare disease. Ho Chi Minh City Children’s Hospital has only recorded about 10 cases. World literature only recorded a few scattered cases.

At present, the child’s condition is gradually improving, awake, rosy, and continues to be treated at the Infection Department.

The cause of acute laryngitis is usually infection with the parainfluenza virus (parainfluenzae virus), which occurs in children aged 5 months to 3 years, but nCoV can also cause this condition. It is suspected that the child may be coinfected with influenza and COVID-19 at the same time.

However, Vietnam does not do diagnostic tests for influenza, because flu is less fatal. Therefore, it is not possible to accurately determine the causative agent of acute laryngitis, whether the boy was coinfected with influenza and COVID-19.

Normally, the larynx is a vocal organ located in front of the neck, an anterior protrusion, also known as the “forbidden fruit”, has the function of pronunciation. When the larynx is inflamed, the child has a fever, speaks, cries with a hoarse voice, coughs and sounds hoarse.

If the swelling is much edematous, obstructing the airway, leading to the child showing difficulty breathing, cyanosis, laryngeal stridor when inhaling, leading to death if not treated promptly. Through this case, parents need to pay attention, when the child has a fever, cough, hoarseness, parents should be vigilant and take them to a medical facility for a doctor to examine to find the cause and timely treatment.
