Liver cancer has a very high mortality rate due to its silent progression, most of which are detected late. The most important thing in the treatment of liver cancer is early detection.

According to GLOBOCAN 2020 statistics, the frequency of liver cancer in Vietnam is quite high. Especially in men, liver cancer occupies the leading position in all types of cancer. In women, liver cancer is the third most common type of cancer. The rate of liver cancer in men is four times higher than in women. The mortality rate of liver cancer is comparable to the detection rate.

“Vietnam belongs to the epidemic area where hepatitis B and C viruses are the main cause of liver cancer. Therefore, the rate of liver cancer in Vietnam is quite high. Therefore, it is important to prevent cancer. The liver needs to take preventive measures to prevent progression to cancer.” Dr. Hoang said.

According to Dr. Hoang, not everyone gets liver cancer. Liver cancer is at a higher risk in people with chronic liver disease. The two diseases with the highest risk of liver cancer are hepatitis B virus and hepatitis C virus.

In which, hepatitis B virus has the highest frequency of causing liver cancer. Especially in men over 40 years old; There is a family history of liver cancer.

In people with viral hepatitis with a high viral load of more than 2000IU/ml, co-infection with other viruses (HIV, HCV), xowgan, the risk of developing liver cancer is 100 times higher than that of the general population.

Liver cancer also has a high incidence in people with cirrhosis due to alcohol or any cause. In addition, autoimmune hepatitis and cholestatic cirrhosis also increase the risk of disease.

Dr. Hoang recommended that recently in Vietnam there is an emerging disease that is fatty liver. People with fatty liver, if not diagnosed and treated early, can lead to cirrhosis progressing to liver cancer.

Certain toxins such as aflatocxin from fungi in cereals and toxic fumes from smoking also increase cancer risk factors for people with pre-existing liver disease.

Liver cancer is often detected late

Dr. Hoang said that most cases of liver cancer are detected late. This is because liver cancer often arises and progresses silently without symptoms in the early stages in patients with chronic hepatitis B and C or cirrhosis. By the time of detection, the patient is usually cancer. Most patients are diagnosed when there are complications of cancer. For example, the patient has pain in the right lower quadrant or the patient has difficulty eating…

In Asia, the incidence of liver cancer and cirrhosis in subjects was low before 35-40 years of age, but then increased exponentially.

Can cancer be detected early? Doctor Hoang said that although liver cancer progressed silently, it took time. For example, tumor growth from 1cm to 2cm must take at least 100 days. Therefore, periodic monitoring will soon detect liver cancer. In the group of high-risk patients, doctors recommend screening every 3-6 months.

According to Dr. Hoang, to detect liver cancer, you can do an abdominal ultrasound test, which is not too expensive. For high-risk people, the doctor will order an additional test of AFP secreted by cancer cells.

“Hepatic cancer at an early stage can be cured by surgery, destroying the tumor with high-frequency waves… When the tumor is too large 10cm or more, occupying most of the liver, treatment will be expensive, easy to recur, and must use other drugs. combined measures, but the effect is not as expected”, Assoc. Hoang said.

Gastroenterologists say the best way to prevent liver cancer is to go for regular health checkups. If liver disease is present, it must be completely treated and managed so that it does not progress to liver cancer.
