Post-Covid ‘Omicron era’ – don’t worry too much

Dr. Le Thi Thanh Thuy said that the concept of “post-Covid” is currently not well understood, so many people are worried and confused. According to Dr. Thuy, experts in the world said that there are 3 criteria to diagnose a person with a post-Covid condition: First, being infected with Covid for more than 3 months; the second is having both physical and mental symptoms lasting more than 2 months; Third, these symptoms cannot be explained by another diagnosis.

From July 2020 to early 2022 in the world, there are more than 900 scientific articles talking about post-Covid, but that’s only the post-Covid of the Alpha, Delta variant. The Omicron variant has not had enough time to publish post-Covid studies, so the patient’s persistent symptoms after contracting the Omicron strain are collectively known as Covid not yet recovered.

General symptoms of post-Covid: fatigue, shortness of breath, prolonged chest pain… However, according to the observation and summary of the epidemic situation in Vietnam as well as in the world, it can be seen that variations Omicron causes much milder symptoms than previous strains.

As a doctor participating in the treatment of pediatric patients infected with Covid from the time the Delta variant “raged” in Ho Chi Minh City, until the Omicron strain “self-made” in Hanoi and many provinces. In another city, Dr. Do Tuan Anh said that from February 2022 to now, children infected with Covid mainly have the Omicron variant.

Like many studies that have been published by foreign scientists, the Omicron variant of Covid 19 increases the number of infected children, but up to 40% of children have no symptoms, 44% have mild symptoms, so symptomatic treatment is recommended. Nutritional support is important.

Assoc.Prof.Dr. BS. Hoang Bui Hai also agreed with the comments of Dr. Le Thi Thanh Thuy and Dr. Do Tuan Anh. During the treatment of Covid patients, he found that many Omicron mutant Covid infections had no symptoms, or mild symptoms. “Even the complications, if any, are not much different from those of the common cold listed by Oriental Medicine,” he said, noting that as long as the patient focuses on nutrition and lifestyle, over time the symptoms will change. This symptom will go away. However, according to doctors, if a patient has severe symptoms (usually infants, the elderly, people with underlying medical conditions) they need to be examined and treated aggressively.

Treatment of pediatric patients

Doctor Do Tuan Anh said that in pediatric patients, in addition to respiratory and digestive symptoms like Delta/Alpha strains, children with Omicron variant Covid have up to 30% of symptoms in the upper respiratory tract with common complications. is acute laryngitis, acute bronchiolitis in infants under 6 months of age, and it is the main reason why children are hospitalized and emergency visits.

Regarding Covid treatment for children, it is necessary to follow 4 principles: The first is to classify the severity of the disease, the second is to treat the symptoms, the third is to personalize the treatment measures, the fourth is to monitor the detection. and treat when the disease progresses differently.

The goal of treatment is to be SAFE AND RIGHT. RIGHT: is the right time, the right medicine, the right dose, and the right course. SAFETY: is the rational and correct use of drugs, to avoid drug abuse, increased side effects, and drug interactions. ANTIBIOTICS are not drugs used to treat Covid, unless the patient has demonstrated/suspected symptoms of superinfection.

In the past time, Dr. Do Tuan Anh also regularly consults and educates parents or caregivers of F0 children at home: with monitoring and detecting dangerous/life-threatening signs and symptoms to promptly time to take the baby to the doctor/emergency at the hospital.

Danger signs of children with Covid include 3 groups: difficulty breathing, rapid breathing, purple lips and extremities; dehydration group, common in F0 high fever, with gastrointestinal symptoms such as abdominal pain, vomiting, poor/anorexia; and the whole body group (lethargy, anorexia, pale/purple skin, persistent high fever, convulsions, etc.)

Doctor Do Tuan Anh also emphasized that children, as well as adults, often need time to recover after being infected with Covid. To support this process, parents or caregivers need to pay attention to the child’s nutrition and rest. Note for children to use nutritious, easy-to-digest foods and drinks; Limit your exposure to electronic devices outside of school time.

Treatment of adult patients

Doctor Hoang Bui Hai said, the post-COVID time in adults lasts 2-12 months. Recovery time for mild disease is about 2 weeks, severe disease is 2-3 months.

For most patients with mild disease who have recovered, testing is not needed. For patients recovering from severe illness, patients with identified abnormalities, patients who have been discharged from the hospital, or who have continued unexplained symptoms, the following tests should be performed: Complete blood count, Blood biochemistry, Other specialized tests: NT-proBNP, troponin ; D-dimer; Thyroid test; Creatinine kinases.

In addition, depending on the severity, the doctor will order an electrocardiogram, chest x-ray, computed tomography lung, echocardiogram, respiratory function measurement, arterial blood gas, 6-minute walking test .

Next, with common symptoms in people with post-Covid symptoms such as difficulty breathing, persistent cough, prolonged severe chest pain, orthostatic autonomic dysfunction, other neurological and cognitive sequelae. , increased blood clotting/thrombosis; changes in smell, taste; fatigue, reduced stamina… Dr. Hoang Bui Hai also gave specific examination and treatment instructions for patients, so that each person, depending on the symptoms of the body, will discuss with the doctor and choose. receive better treatment.

Doctor Hoang Bui Hai also shared the role of post-Covid rehabilitation. Accordingly, he believes that patients with a need for rehabilitation services should proceed as soon as possible, usually within 30 days of recovering from COVID-19. All patients should be screened for cardiac symptoms prior to initiating any exercise program.

Giving detailed instructions for patients who are worried about post-Covid symptoms, the doctors participating in the workshop emphasized: “People with Covid at this time should not be too worried and confused. Eat and drink. enough, exercise moderately, to recover health soon, rest assured to return to life as usual.”

In the question and answer session at the end of the program, Assoc.Prof.Dr. BS. Hoang Bui Hai, Dr. Do Tuan Anh and BS.Dr. Le Thi Thanh Thuy answered questions and advised treatment directions for many viewers about the symptoms that they or their loved ones are suffering from after contracting Covid 19.
