A man who vomited violently after a drinking party to the point of bursting his esophagus has just been treated by doctors in time

On the morning of April 28, the 115 People’s Hospital of Ho Chi Minh City said that it had just received treatment for a man who had a rare accident: ruptured esophagus after a drinking party.

This 46-year-old male patient was admitted to the emergency room with abdominal pain above the navel, spreading throughout the abdomen, epigastric pain spreading to the back so much that it was difficult to breathe but could still urinate. The patient vomited many times before after drinking at home.

At the hospital, the initial diagnosis was to monitor acute pancreatitis, not excluding gastroduodenal perforation. The CT-Scan results showed fatty infiltrates in the right lower half of the abdomen, which was not clear whether it was mediastinal gas or gastric hernia.

The patient was transferred to the Department of General Surgery for monitoring and necessary further tests. The results of gastroscopy showed that the lower esophageal cardia was torn, the tear area was wide, high, and the bottom was deep, not ruling out perforation.

Immediately, 2 teams coordinated with the Department of General Surgery (MSc.BS.CKII Bui MinhThanh) and Department of Thoracic Vascular Surgery (MSc Nguyen Minh Luan) decided to operate. When opening the patient’s left chest, the left central angle of the diaphragm showed many white pseudomembranous membranes, cloudy effusion, an inflammatory mass in the mediastinum posterior to the esophagus extending a distance of 20cm from the diaphragm to the left hilum of the lung, the collection of pus closed. cocoon.

The doctors dissected, enlarged, washed the abscess and placed in the thoracic cavity an irrigation system along with the left pleural drain. Examination of the esophagus revealed a 3 cm long laceration.

The patient was surgically sutured to close the esophageal perforation, covered with a large omentum on the suture, placed 2 more drains and opened the stomach for feeding.

After surgery, resuscitation, the patient is now in good health, breathing on his own, and able to communicate.

According to doctors, this is one of the rare cases of esophageal rupture and the risk of serious complications if not diagnosed and treated promptly.
