Consecutive cases of testicular torsion

According to information from the Department of Andrology and Sex Medicine at Hanoi Medical University Hospital, in recent days, the department has continuously recorded cases of emergency surgery due to testicular torsion, including cases due to subjective reasons. was late to the hospital and had to have his testicle removed. Worth mentioning, these are all very young cases, in their teens, with a long future ahead of them.

Typically, the case of a 16-year-old male patient in Hanoi, one day before admission, suddenly appeared severe pain in the left testicle. After that, the patient felt less pain, so he subjectively did not go to the doctor.

By the time I went to the doctor, the testicles had begun to show signs of necrosis on ultrasound and laboratory tests. Although emergency surgery was indicated, the testicular lesions were not reversible and the left testicle was required to be removed.

Another case was an 18-year-old male who had severe pain in his left testicle in the middle of the night. Because he was afraid to go to the emergency room, the patient was admitted to the hospital early the next morning. Fortunately, during the clinical examination, the doctors promptly detected testicular torsion and ordered emergency surgery. Because of early and timely surgery, the patient’s testicle was saved.

It is not difficult to detect testicular torsion

Doctor, Doctor Nguyen Hoai Bac – Head of the Department of Andrology and Sex Medicine at Hanoi Medical University Hospital said that when experiencing the following symptoms, you should think of testicular torsion:

– Sudden, severe, acute pain in the testicle on the side of the torsion.

– Visual inspection can see the testicle rotated horizontally and in a higher position than normal.

– Patients often feel more severe pain when lifting the testicle on the affected side. This is a significant finding to differentiate torsion from orchitis.

However, there are also cases where the patient only has a dull ache, leading to psychological concerns about examination.

Doctor Bac warned, without timely intervention, irreversible damage to testicular structure and function will begin to appear after 4-8 hours. Therefore, for patients with testicular torsion, time is gold.

Accordingly, the ability to “save” testicles is quite high for patients who arrive before 6 hours, up to 90-100%. The rate of successful testicular preservation falls below 50% if the patient arrives after 12 hours and especially less than 10% if the patient arrives after 24 hours.

Testicular torsion is common in young people

According to doctor Nguyen Hoai Bac, acute testicular pain is a fairly common and typical symptom in men. One of the most common causes of this condition is testicular torsion.

However, it is the subjectivity and fear of the patient as well as the parents that lead to the delay in treatment. The end result leads to necrosis and the need to remove the testicle.

The testicles are nourished by the blood vessels that run in the spermatic cord. Torsion occurs when the testicle rotates around its axis leading to acute ischemia. The disease can occur at any age, but testicular torsion is mainly seen in two main age groups: infancy and puberty, especially before the age of 18.

In the case of a 16-year-old male student above, the loss of one testicle has little direct impact on the general health of the body, but can have a serious impact on sexual and reproductive health. Besides, the fact that only one testicle remains has a great influence on the psychology of patients, especially in young people.

Therefore, patients need to be very vigilant, should not be subjective when seeing abnormal signs in the testicle area, especially acute pain to avoid unfortunate consequences later. Seek medical attention immediately if you have any of the above-mentioned signs suggestive of testicular torsion. At the same time, it is necessary to choose a reputable facility with a specialist in andrology for timely examination, diagnosis and treatment to avoid unfortunate events.
