Mr. Tran Ngoc Hong (93 years old, from Vung Tau) returned to Vietnam after a trip to India in early May 2022. Seeing that he was tired, his family thought that because of his old age, he went on a long trip. A few days later, he began to feel weak on the sides of his body, had difficulty speaking, had a cough with cloudy sputum, was constantly thirsty, and urinated many times. As his symptoms increased, his family took him to the emergency room at Tam Anh General Hospital in Ho Chi Minh City.
High blood sugar level 700 mg/dl
Receiving patients, MSc Phan Thi Ngoc Loi (Emergency Department, Tam Anh General Hospital in Ho Chi Minh City), said that the patient was admitted to the hospital in a weak condition on the right half of the body. Patients are prescribed MRI scans, X-rays, blood tests, and urinalysis. The results recorded that the patient had type 2 diabetes with a very high blood sugar level of 700 mg/dl, pons damage suspected due to increased blood osmotic pressure. While a person’s normal blood sugar level before eating ranges from 90-130mg/dl and after eating is 180mg/dl.
The patient also had pulmonary tuberculosis on the background of diabetes. Doctor Loi prescribed fluid resuscitation and intravenous insulin injection for the patient, and at the same time coordinated with doctors specializing in Endocrinology, Respiratory Department… to continue monitoring and treating.

As soon as he received the results of diabetes, Mr. Hong’s family was quite surprised, because so far, he ate healthy, and did not show any signs of disease. After returning from India, he still ate normally but was thirsty a lot, urinated many times, lost weight, and became increasingly sluggish.
Vo Tran Nguyen Duy (Department of General Internal Medicine) said that the patient has type 2 diabetes, now has complications to increased blood osmotic pressure, taste disorder. If the patient does not get to the hospital in time, the patient can die. After treatment in the Emergency Department, the doctor continued to administer insulin intravenously. Currently, the patient is out of danger and is being treated for pulmonary tuberculosis in parallel with blood sugar adjustment.

Dr Duy said that diabetes is a common disease that develops silently but causes many health consequences. Acute complications of diabetes are ketoacidosis – a condition of hyperglycemia, increased blood ketones and metabolic acidosis with manifestations such as nausea, abdominal pain, coma…
According to Dr. Duy, many cases of diabetic ketoacidosis, if they do not get to the hospital in time, will easily die at home or have a cardiac arrest on the way to the hospital. If life is saved, the patient is also often brain dead or has reduced neurological function. Many people do not know they have diabetes until they fall into a coma and have to be hospitalized in an emergency like the case of patient Hong. When taking the medical history, they realized that they had ignored the symptoms of the disease.
Warning signs of diabetes
According to Dr. Lo, there are three types of diabetes namely type 1 diabetes, type 2 diabetes and gestational diabetes. Among them, type 1 diabetes and type 2 diabetes are the most common.
Type 1 diabetes (insulin dependent diabetes) occurs because the pancreas produces little or no insulin to produce energy for the cells. The disease is common in young people under 30 years old, manifested by symptoms such as frequent urination, blurred vision, thirst, frequent hunger, fatigue, rapid unexplained weight loss. If not detected and controlled in time, type 1 diabetes will lead to complications of ketoacidosis, death. Patients with type 1 diabetes are treated with insulin injections.
Meanwhile, type 2 diabetes (non-insulin dependent diabetes) is a disorder of carbohydrate metabolism caused by the body not using insulin properly or insulin not working properly. Symptoms of type 2 diabetes are similar to type 1 diabetes but progress for a long time, such as: frequent urination, frequent thirst, dry mouth, eating a lot but still hungry quickly, unusual weight loss or gain, kidney failure , edema… Doctor Li said, type 2 diabetes accounts for about 75-85% of all diabetes cases.
Based on the degree of type 2 diabetes, the doctor will prescribe appropriate treatment to control blood glucose. Patients take diabetes medication and adjust their lifestyle, eat more green vegetables (such as cauliflower, broccoli, squash…), limit fat and protein, cut calories, exercise By walking, doing gentle movements… In case the drug is not effective, the doctor may appoint the patient to combine diabetes treatment with insulin or complete treatment with insulin.
According to estimates by the World Diabetes Association (IDF) in 2021, there will be about 537 million people with diabetes. It is expected that this number will increase to 643 million people in 2030 and 783 million people in 2045.
In Vietnam, according to the Vietnam Association of Endocrinology and Diabetes in 2021, there are more than 3.5 million people with diabetes with more than 55% of patients having complications. In there, 34% had cardiovascular complications; 39.5% had eye complications and neurological complications; 24% kidney complications.
