Men with secondary asexuality, testicular atrophy, no spermatogenesis, what to do to have children?

Secondary infertility for decades, atrophy of the testicles, no spermatogenesis… these are contrived situations that make men think they can’t have children.

Happiness burst

According to statistics of the World Health Organization (WHO), Vietnam currently has about 7.7% of infertile couples, equivalent to one million couples. It is estimated that about 50% of infertile young couples are under the age of 30. In particular, the rate of primary infertility is more common than primary infertility, the rate of male infertility is often higher than that of females. increase every year.

Talking to reporters on the morning of May 29, MSc Dinh Huu Viet, Head of Department of Urology and Andrology, Hanoi Hospital of Andrology and Infertility, said that there are many couples who are infertile for reasons that are not doubt. And in that long journey to find children (with couples up to 18 years) there were … “sweet fruits” born.

Desperately infertile men still give birth thanks to these techniques
Tran Khac Dat and his wife are happy with their daughter

That is the case of Tong Thi Thu Ha (SN 1981), Mr. Tran Khac Dat (SN 1974) in Phu Tho. Brothers and sisters came together in 2016 when both had experienced a boat trip, determined to live together to rely on each other in old age. At that time, he had been suffering from secondary infertility for 23 years, each with their own children.

However, events kept happening to the family. In 2017, Ha discovered that her daughter had a brain tumor. A year later, her daughter died, she broke down.

“My mother and I have lived together for 14 years, but now when I find someone to rely on, I leave her,” Ha said chokingly.

My wish at that time was to open a fast food restaurant. Therefore, she rushed to work to forget the pain of losing her child. When she got home from work, she was too tired to come home and lay her back down and she didn’t know what to do.

Things did not stop there, she had an accident with a broken sternum and had to be hospitalized. During her hospital stay, she watched TV and saw a couple in Bac Giang who had the same condition as her husband but still had children.

So she rekindled a hope even though it was fragile. She began to learn about the treatment of a similar case with her husband. In 2018, the couple decided to place IVF. Fortunately, the first time was successful, despite countless difficulties. .

In 2019, “sweet fruit” Tran Ngoc Phuong Diem was born in boundless happiness.

“Unbelievable, 47 years old I am a father again. I’m very happy, because I myself have had complicated mumps (when the first wife was 6 months pregnant), testicular atrophy, could not have children, secondary infertility for 23 years”, Mr. Khac Dat held his child in his arms and said happily.

Or another family, Ms. D. T. T – Mr. MNN (Nam Dinh). Mr. N. has Klinefelter syndrome (males with this syndrome have one pair of X chromosomes instead of only one X chromosome, have chromosome set 47XXY (normally 46XY) with the characteristic Characterized by hypogonadism, endocrine disorders, testicular atrophy, poor spermatogenesis or no spermatogenesis) it seems impossible to have children.

Thanks to the surgical intervention of microscopic testicular surgery to find sperm and perform in vitro fertilization with his wife’s eggs, he and his wife were finally happy to welcome the baby after many years of trying to cure it.

Before that, during the treatment process, both husband and wife also fell into crisis despite psychological preparation. However, neither the patient nor the doctor gave up. The doctor had to use a microscope, trace each layer in the testicles, and fortunately “catch” some sperm for IVF to help this couple have a baby girl later.

The house that is always filled with laughter also comes to the family of Ms. Pham Thi Bich and Mr. Nguyen Quoc Hung (Lai Chau) after 13 years of searching for children with many concerns. Economic difficulties, brothers and sisters used to think “rescue” each other. But the wife’s perseverance, her husband’s determination kept them together and they had a lovely little daughter.

Mentality determines 50% of the chance of success

“We sincerely advise patients who have infertility for any reason to try to stay optimistic because mental factors can contribute up to 50% of the chance of treatment success.” , BS Viet said.

Doctor Viet said that men with infertility caused by blockage can have microsurgery to remove the blockage and reconnect the vas deferens and epididymis.

In the absence of sperm due to poor testicle production, microsurgery can be performed to “catch” each sperm and inject it into the egg cytoplasm to form an embryo and then transfer it to the woman’s uterus.

For the case of azoospermia due to vas deferens, Dr. Viet said that the patient will be applied the percutaneous epididymal aspiration technique. This technique is quite simple, can be poked through the skin and usually will get a lot of sperm because the spermatogenesis process takes place normally, only the vas deferens is blocked…

In the case of testicular damage due to mumps, testicular atrophy, poor spermatogenesis, higher techniques such as Micro TESE – testicular tissue analysis are often needed to find sperm. This technique is quite new in Vietnam and is also considered a last resort to find sperm for azoospermia patients. The doctor “unearths” the entire testicle to find each sperm. When finding sperm on such small tissue samples, filtration is also very difficult, requiring meticulousness.

Through these cases, Dr. Viet recommends that, when examining, if the patient still has sperm, the doctor advises to take frozen sperm for later use as a reproductive aid. If the patient does not have sperm, the technique of collecting sperm from the epididymis and preserving it will be applied. When a couple wants to have a baby, frozen sperm will be thawed, performing in vitro fertilization.

Especially to prevent mumps, families with boys should have their children vaccinated against mumps (because mumps can atrophy both testicles), vaccinate children against tuberculosis, and keep them clean and healthy. urethritis.
