SIS Can Tho International General Hospital successfully saved the life of a patient with a ruptured cerebral blood vessel while singing karaoke and suddenly had a severe headache.

Ms. NTV (66 years old, native of Bac Lieu) has a history of high blood pressure, for more than 1 year now she is often dizzy, has taken antihypertensive drugs but does not comply with “when increasing, drink, when down, stop”. Recently, during a karaoke session, something very rare happened: a severe headache that was diagnosed by a doctor as a ruptured blood vessel in the brain.

After having sudden symptoms of headache, Ms. V. was taken to a local hospital by her family for 3 days, but the headache did not decrease but increased. Mrs. V. realized that her condition was getting worse, so she asked to be discharged, and her family took her to SIS Can Tho Hospital.

According to Ms. V. on the way to Can Tho SIS Hospital, she had a lot of vomiting, with a more severe headache than 2 days before. At the hospital, Mrs. V. was examined by doctors and had a CT scan to find out the cause. The CT results showed that Mrs. V had a subarachnoid hemorrhage due to a ruptured cerebral aneurysm.

The doctors of Can Tho SIS Hospital conducted a consultation to decide on surgery, in order to promptly handle and help Ms. V. escape the life-threatening danger.

Dr. CKI Nguyen Quang Hung – Head of Neurosurgery Unit of SIS Can Tho Hospital – The person who directly operated on Ms. V said: “In this patient, it can be said that it is extremely lucky, most likely in the pulse point. Broken blood has too thick atheroma, so it can stop the bleeding temporarily, so the patient has a chance to go to SIS Can Tho hospital after 3 days of rupture.

For an aneurysm with large size, wide neck, heavy bleeding causing brain compression, when DSA endovascular intervention is difficult like this case, we can surgically clamp the neck of the aneurysm to stop the bleeding and remove the bleeding. The hematoma has drained around the aneurysm, and it is important to preserve the surrounding normal blood vessels…”.

After 5 days of surgery, Mrs. V.’s health was stable, recovered well, with less headache, no weakness. Mrs. V. recalled: “I stayed at home to raise my sick husband, my husband also liked me so he also started taking care of me. Singing for him is less sad, passionate, so going to the party also registers for a musical performance. That day, when I sang old songs, I didn’t finish verse 1 anymore, I heard a pop in my head like beating a desiccated coconut, then it went dark …”.

Dr. Hung assessed that Mrs. V’s aneurysm may have been at least 1 year before, because the size of the aneurysm is very large, so when the patient sang, she might have exerted herself in the climax leading up to the climax. Blood pressure increases, causing blood flow to increase too much beyond the limits of the aneurysm, causing the aneurysm to burst.

Currently, Mrs. V’s health is stable after surgery, no bleeding, no seizures, no weakness in limbs and has been discharged from the hospital. According to the doctors, Ms. V needs to have regular check-ups for specialists to re-evaluate whether there is recurrence of aneurysms, excess neck or not…

Through this case, Dr. Hung also recommends that middle-aged patients with high blood pressure, headaches, drooping eyelids, stiff neck… should be screened for cerebral aneurysms at least once by MRI but at least once.
