Hospitals are difficult, patients are suffering

Part I: Lack of medicine people “turn away” from the medical station

Go for medical check-ups with health insurance (health insurance) again lack of medicinehaving to buy medicine from outside is the reason why many people are “bored” at the grassroots health station, making many medical facilities “lonely”.

Lack of medicine because 10 drugs health insurance is only given 1-2 types

The Medical Station of Ward 4, District 10 (HCMC) currently manages thousands of people in the area. This is one of the medical stations eligible for initial health insurance examination, however, because of the increasingly poor medicine cabinets plus inadequate facilities and human resources, the number of patients coming for examination is getting smaller and smaller.

Pharmacist Le Minh Nguyet – medical station in Ward 4, District 10 said that the drug source of the station is usually allocated by District 10 Hospital. The station proactively proposed a list of drugs to receive, however, instead of suggesting that 10 drugs will be approved or missing 1-2 types, now 10 drugs are only given about 1-2 types. The reason given by the hospital was that the drug could not be tendered.

According to pharmacist Le Minh Nguyet, many people visit the health station but do not have any medicine or are given a different medicine than the one they received at the hospital, so they refuse to receive the medicine, then they also do not visit the station always.

This situation also happened at the medical station of ward 15, Phu Nhuan district. The shortage of medicine cabinets before and after made the doctors at the station, even though they wanted to examine and give medicine to people, they had to endure the battle.

The most difficult is the medical stations in Thu Duc City (HCMC). Most of the medical stations here have been empty of drugs under the health insurance list for many months now. BSCK1 Lam Thanh Huong, Head of Professional Planning Department – Medical Center of Thu Duc City, said that since 2016, medical stations are eligible to carry out initial health insurance examinations for people.

By the end of 2020, medical stations will stop examining. “In order to check health insurance, the hospital must support personnel for the health station. Doctors must have a practice certificate in health insurance examination and prescription at the health station. However, these doctors are all working on a full-time basis. At the hospital, the hours of medical examination at the hospital registered on the general portal of the industry coincide with the working hours at the medical station, so they are not eligible for payment. so far,” said Dr. Huong.

Currently, in Thu Duc City, there are only 2 wards that maintain health insurance, which are Binh Chieu and Hiep Binh Chanh wards. The reason is that there is a satellite polyclinic of Thu Duc City Hospital located at these two medical stations, all personnel, equipment, and medicines at these two facilities belong to the hospital, the medical station does not get involved.

Dr. Huong added that during the period when the health stations were still performing health insurance checks, the number of patients coming for examination varied from a few dozen cases to more than 100 cases/month depending on each station. However, now, when there are no more drugs on the list of health insurance, patients almost no longer come to the station for examination.

Not only are there no conventional medicine for medical examination and treatment at level 4, the health stations are also in a state of not being able to allocate drugs to treat chronic non-communicable diseases, and cannot perform paraclinical tests.

There are no more pills according to the health insurance list

Doctor Tran Van Quy, head of the medical station of Hiep Phuoc Ward (Thu Duc City) pointed to an empty medicine cabinet and said: “The station does not have a single pill left under the health insurance list, currently only a few emergency drugs are left. self-equipped by medical stations and centers.

There is no medicine, so people no longer want to visit here because after the examination, the doctor can only prescribe for the patient to go out and buy medicine by himself. So they went straight to the hospital to check it out.

If enough medicine is provided, I believe people will still come to the station because a lot of people have common illnesses such as flu, cough, fever, diarrhea, arthritis… they want to be examined at the station closer, so they don’t have to go far or jostle. queue at the hospital”.

Currently, the medical station of Hiep Phuoc ward still examines people in need and dispenses drugs according to national programs such as tuberculosis, neurological disease, HIV …

Lack of medicine because there is no contractor to supply medicine to the medical station

Talk about the problem lack of medicine At health stations, Ms. Le Thien Quynh Nhu, Deputy Chief of the Office of the Health Department of Ho Chi Minh City, said that during the Covid-19 epidemic period, medical stations mainly performed disease prevention and control and Monitor, manage and consult F0 patients treated at home.

Currently, the situation of the Covid-19 epidemic has been controlled, so the medical stations returned to the initial examination of patients with health insurance. During the transition period, there was a shortage of certain drugs in the medical station’s supply of drugs.

Specifically, the health station is a class 4 medical examination and treatment facility, according to regulations, it will be provided with a list of drugs according to class 4 in Circular 30/2018/TT-BYT of the Ministry of Health, so there will be a shortage of some drugs when Chronic patients are transferred to health stations from 1st and 2nd class hospitals in the city.

Meanwhile, the health stations have not yet approved the technical list and the list of superior drugs according to regulations.

Besides, at present, the number of people visiting the health stations is not much, so the procurement of drugs at the health stations is also facing difficulties because there are no contractors involved in the supply of drugs.

To resolve the situation lack of medicine At the medical station, the Department of Health held a meeting with medical centers and health stations, providing information on the implementation of the list of drugs beyond the route at the health stations, the drug procurement process,…

However, in the long term, procurement of drugs at health stations should be done in the direction of centralized procurement to ensure the selection of contractors involved in the supply.

According to Ho Chi Minh City Social Insurance, up to now, there are 190 medical stations in the area that have signed contracts for medical examination and treatment with health insurance. In 2021, 125 medical stations will have medical examination and treatment activities with 33.8 million visits. In which, the health insurance fund pays 190 billion VND, an average of 168,000 VND/time.

In the first 5 months of 2022, only 101 medical stations have medical examination and treatment activities with 13 million visits, on average, health insurance pays 200,000 VND/time. The total amount paid by health insurance to health stations is very small, accounting for only 0.1% of the total expenditure in the area.

Ho Chi Minh City Social Insurance said that, in order for the health station to attract people for medical examination and treatment, it is necessary to develop a list of drugs, especially a list of drugs that are out of line to provide timely and adequate service to patients’ needs. . However, the list of drugs must be close to reality, avoiding excess and wasteful inventory.
