COVID-19: Why is China suffering from its most severe COVID-19 outbreak since Wuhan?

The 4th outbreak unfolding across the country has caused difficulties and burdened the health system, especially the grassroots healthcare sector. Since Tet, the number of COVID-19 cases has increased dramatically, spreading rapidly across the country by the Omicron mutation. Hanoi leads the country in the number of new cases, daily recording more than 30,000 F0, of which 99% of them are treated at home. The pressure is on the shoulders of the grassroots medical system when the entire medical station only has 8-10 medical staff, while they have to be responsible for thousands of F0s. In recent days, many doctors and medical staff have been infected with SARS-CoV-2, in some places the entire F0 medical station, difficulties are piling up.

No holiday

We went to the Dong Tam Ward Health Post, Hai Ba Trung District on a stressful day when the COVID-19 epidemic in Hanoi broke out. Every day there are more than 100-200 new positive patients. For nearly half a year, the work of grassroots medical staff was too busy, tired and even more burdensome as F0 cases began to rise sharply in the capital after the Lunar New Year.

Call Dr. Nguyen Minh Hue, Head of the Medical Station, heard a cough on the phone. I asked, he said: “Today is the 7th day of being infected with SARS-CoV-2″. In the midst of the most stressful epidemic, all medical stations have 10 people (1 doctor as station head and 9 nurses and nurses) infected with COVID-19. “It is very difficult when all stations are F0. In that situation, I encourage the sisters to try, still receive counseling for F0s through Zalo, who doesn’t have a fever, goes to the work station,” said Dr. Hue.

Dr Hue said he had COVID-19 with all the symptoms, cough, malaria, sore throat… but was still being directed from afar. When the fever stopped, there was a phone call, he answered the phone to advise the patient. “There are tired nights, sleeping, receiving news, still waking up to advise and direct. There was a sleepy afternoon, people kept drowning. But then I thought, I have to work on forgetting that I am F0. Busy working from morning to night, no time to think about illness, feeling better. I encourage the F0 sisters to try to get through this difficult time. Love for work conquers all, we work around the clock as F0, forgetting the symptoms of illness”, said Dr. Hue.

Apart from Dr. Hue, the child has grown up, 9 nurses and nurses from Dong Tam Ward Medical Station have many young children. Medical personnel with fever and small children did not come to the station, but all worked from home and received counseling through Zalo. Those who are stronger go to the work station on the 2nd floor to handle administrative work and support F1 downstairs (F1 is Youth Union, Medical Faculty students).

“Each medic is on duty in the residential area, whoever is too tired, the stronger one will do it, sharing the work with each other. I prepared an instructional message of treatment steps, using medicines for F0 at home. This message is sent to every F0 patient by staff, so in the past, there have been no serious transfer cases that had to be transferred late,” said Dr. Hue.

The grassroots health service in Hanoi is entering a human resource crisis because each medical station has only 8-10 employees, the prolonged epidemic makes their workload overloaded, they are busy every day. Busy, no holidays. Many people do not have time to take care of their families, there are sick people at home, small children, COVID-19 infection and do not have much time to take care of them. Not to mention, the outbreak after the Chinese New Year so far, the number of medical staff infected with SARS-CoV-2 is very high.

Not only Dong Tam Regional Medical Station, Hanoi has many places where both medical stations are F0 such as Thanh Xuan Bac ward, Trung Van… In addition, many other health stations have medical staff infected with COVID-19, In many stations, infection rate reaches 50-60%, even 90% like Mai Dong ward. At Bangsal Buoi Health Post, when many medical staff were infected with SARS-CoV-2, all personnel had to be mobilized and trained for the ward People’s Committee staff to take samples for COVID-19 testing for people at the health post. mobile.

Night emergency case

“Hello! Please come down to help my family, my mother refuses to go to the hospital.” The call from the 80-year-old woman’s daughter asked for help from Dr. Huh. Over the phone, he persuaded for a while, but the old woman still insisted: “I am a doctor, I have an oxygen tank at home, I don’t need to go to the hospital” … This is a midnight phone call that Dr. Hue often accepts, especially in the “difficult” time when the entire medical station is F0. The old woman had cancer, her illness turned serious, but her family persuaded her not to go to the hospital. In this case, Ms Hue had to go on the scene to persuade, analyze right and wrong, and even “threatened” her being admitted to the hospital.

Recently, there were two old grandparents living alone, after being infected with COVID-19 and being treated at home, he had a background disease, every day the children brought rice and hung it on the door. On the 4th day, the couple was tired, his blood sugar was low, he passed out in the toilet, he called the ward medical center. Doctor Hue and a medical staff rushed to the place, “broke down the door” to enter the house, gave him oxygen, ate porridge, drank milk, after a while he regained consciousness.

Dr Hue said that in the process of receiving, consulting and treating F0, there were many stories of “bad crying and laughing”. There were parents with serious underlying illnesses who tested positive and refused to go to the hospital. On the first day when he was found positive, he was given antiretroviral drugs, and he was taking them very well. But on the 3rd day, his illness got worse and he was hospitalized, 1 week later he died. There are women who refuse to take medicine, refuse to eat, their children call for medical personnel, have to “coax”, even “threaten” to eat. There are medical personnel who have to call every day to persuade “already eat, drink milk, eat lots of rice to get well soon” like persuading children.

Or there are phone calls that the sisters just receive, the other party says “why don’t you come to test me at home”. “At that time we all had to explain, the public understood and loved medical personnel. There was a 90 year old man who was immediately transferred to the hospital due to an emergency. When he recovered from his illness, his son called the station to say thank you: “Miss Hue, thank you, my father has returned from the dead. .” When we found out that all posyandu were F0, people came out to give us bananas, cookies, nuts… We were very touched,” said Dr. Hue.

According to statistics, from January to now, Dong Tam ward has recorded 2,000 cases of COVID-19, 900 of whom have recovered, the rest are being treated at home. Recently, many severe COVID-19 patients in the ward thanks to timely referrals have been rescued, out of harm’s way, discharged from the hospital to their families. But in addition there are also 6 people who died (from January to now) are elderly, have underlying diseases, some have not been vaccinated. Apart from people who are aware, there are still some cases of positive but undeclared tests. Some elderly people only know they have COVID-19 when they die. Or there are people who do not report, until they are seriously ill, already in critical condition.
