Doctors put patients on mechanical ventilation, fluid control, anti-shock, continuous dialysis, plasma exchange, liver treatment. However, the patient’s condition did not improve due to severe multi-organ dysfunction, requiring ECMO intervention. This is an artificial cardiopulmonary system, an oxygen device through the outer membrane of the body, the ultimate respiratory support weapon for critically ill patients.

According to Dr. An, there are very few dengue fever patients assigned to ECMO intervention, but this case, if not used, will result in death. When using this technique, the process of correcting coagulopathy is often difficult while dengue fever causes severe coagulopathy. To handle it, doctors applied a protocol of both ECMO intervention, blood exchange, continuous dialysis, and hourly control of blood clotting.

Patients underwent 10 days of ECMO intervention, 12 times of continuous dialysis, 14 times of plasma exchange with a volume of 56 liters of blood, while most other severe patients only had 3-4 changes. The patient was also transfused with red blood cells and a large number of platelets. After 47 days of treatment, the patient’s liver, kidney, and respiratory function recovered, the laboratory parameters improved well, and he was discharged from the hospital.

Up to now, this is the dengue fever patient who received the most blood transfusion recorded in Ho Chi Minh City.

Dr. Truong Dinh Cam, Deputy Director of Military Hospital 175, said that a very severe case of dengue fever with overlapping septic shock, rocky skin like Mr. Phuoc is often difficult to survive. During the course of treatment for him, there were times when doctors were concerned that the patient’s life could not be saved. “Fortunately, the efforts of the doctors and nurses, the patient’s own will to live, and the encouragement of the family have helped bring about good results,” said Dr. Cam.

From the end of April until now, the number of dengue fever cases has increased continuously in Ho Chi Minh City. In the first 5 months of the year, the city recorded more than 10,000 cases of dengue fever, an increase of more than 46% over the same period last year. There were 19 severe cases, an increase of nearly 5 times compared to the same period in 2021.

Most cases of dengue fever can be treated as outpatients and monitored at primary health care facilities, mainly with symptomatic treatment. When the fever is from 38.5 degrees Celsius, take antipyretic with paracetamol (dose and number of times according to the doctor’s instructions) and combine with continuous cooling.
