‘Nursing is not a doctor’s senior osin’

Many people misunderstand nursing as a nurse or a doctor’s senior osin, leading to this profession not being promoted, according to former Deputy Minister of Health, Professor Pham Manh Hung.

“In Vietnam, the nursing profession has been abolished, leaving only the nursing profession, causing many doctors and people to think that nursing is a nurse, that is, a name,” Professor Hung said at a nursing conference organized by the Institute of Nursing. An Sinh Medical Research and Training was held on May 6. Around the world, the nursing profession is still maintained, besides nursing; Meanwhile, in Vietnam for a long time, the nursing profession is no longer trained.

From the role of an assistant to the doctor, today’s nurse plays the role of coordinating and integrating treatment and patient care. Mr. Hung said that nurses are like “the heart of health care, without nurses, the health care system will collapse immediately”.

Doctors and nurses have different roles and functions. Nursing is concerned with health and restoring health, comprehensively taking care of both the body and the mind of the patient. The doctor focuses on dealing with the disease, such as diagnosing, treating, operating… The nurse’s function is to provide and implement health care techniques and management. , make orders, communicate, participate in training, be a conservator for the patient.

“In fact, doctors and nurses both need to wear a stethoscope to examine patients, but now many nurses don’t dare to wear a stethoscope because they are afraid of being considered a doctor’s job,” said Hung. think that this makes the role of the nurse not promoted, carrying the mentality of “being the doctor’s osin”. In essence, the work of doctors and nurses is independent but supportive, cooperative, and helping each other.

Professor Hung also said that many people do not know the work between a nurse and a nurse. The actual work of nurses and nurses (medical assistants) has many differences, although the similarity is taking care of patients. While a nurse wears a stethoscope to examine a patient, the nurse does not wear a stethoscope. Nurses carry out the orders of both doctors and nurses. Nursing provides the patient’s condition, outlines the care plan, takes the initiative and self-control in patient care. Nurses are trained primarily through practice. Nursing is trained through university.

A nurse can do everything a nurse does, but a nurse cannot do everything a nurse does. The reason is that nurses have the ability to work independently, while nurses often have to work dependent on other titles.

Specifically, nurses take care of the patient, take vital signs, collect blood and urine samples for laboratory testing, deliver samples to and from the lab, help patients meet regulatory requirements. their diets and treatment plans, update and maintain medical records, administer medications, and answer questions regarding the patient’s condition.

Nurses monitor patient response to treatment, make clinical decisions in the absence of a physician, discuss treatment plan modifications, educate patients on alternative treatment programs , coordinate between different departments in the ward, perform physical examination and diagnostic tests for treatment, prepare the patient for surgery or procedure and assist the physician in the process, help the patient personnel perform daily activities such as bathing, walking, eating, receiving and discharging a patient, instructing family members on how to support the patient… Nurses need to detect and monitor symptoms, Implement medical orders and treatment regimens, use medical facilities fluently, do some tests and analyze results under the supervision of a doctor.

According to Mr. Hung, Vietnam’s nursing force currently has many limitations such as lack of quantity. The number of nurses per thousand people is currently about 1.3, while the average of most countries is 3.2 or like Japan up to more than 12, New Zealand, Australia are more than 11. In the resolution issued in 2017 on strengthen the protection, care and health promotion, the goal is that by 2025, Vietnam will have 2.5 nurses per thousand people, which means that the current number of nurses should be doubled.

The ratio of nurses to doctors is about 1.4, lower than the average of many countries in the world, which is 3-4. “Thinking about calculating the number of nurses is based on doctors, an owner who only needs 1.3 osin will lead to such consequences”, Mr. Hung shared.

Regarding the quality of human resources, Vietnam only has general nurses but no specialties, while the world has 20 nursing specialties. Nurses have not properly performed the coordinating function in patient care, have not been able to think critically and independently, and still mainly depend on the doctor’s orders.

Regarding training, although nursing training has been upgraded to bachelor’s and master’s degrees and started to train doctoral degrees in nursing from 2019, the teachers are still mainly medical doctors, so the approach is not the right direction. health care (health behavior) but mainly dealing with disease (disease behavior). The nursing team is less involved in the process of planning and formulating accurate medical in general and nursing policy in particular. There are no long-term orientations and policies for the sustainable development of nursing majors in terms of management, training and use.

Mr. Hung suggested that the health sector should strengthen the construction of the nursing industry, ensuring the quantity, in the immediate future meeting the criteria of 3 nurses/doctor. In addition to general training, it is necessary to renew specialized training, increase the percentage of nursing school teachers who are nurses, and reduce the percentage of doctors who are nursing teachers. Building the functions and duties of nursing in the international direction, considering this a profession with relative independence from the medical profession. “It is necessary to restore the nursing profession, distinguish nurses and nurses, and aim to serve patients in the best way,” emphasized the former Deputy Health Minister.

According to Master Pham Duc Muc, President of the Vietnam Nursing Association and the World Health Organization, nursing services are one of the pillars and backbones of the health service system, but the role of nurses In Vietnam, it has not been properly recognized, it is still a silent profession among the silent professions.

In the 1990s, a series of Vietnamese nurses left the profession because during the 20 years from 1975 to 1995, only intermediate-level nursing training was the highest. Many talented and intelligent nurses have the desire to improve their qualifications, but the door has not been opened yet. Recently, the nursing profession has received the attention of the Ministry of Health, the voice and requirements of the Vietnam Nursing Association, helping the industry to have more achievements, the learning path is more open, nursing can Find a future career. Many nurses have been selected for excellent doctors, participating in national honor programs…

One shortcoming is that in the context of economically autonomous hospitals, which group of professions can create finance for the hospital, that group plays a fundamental role. With the current hospital fee structure, health insurance coverage is absent for nursing services. For example, every time a doctor examines a patient, there is money, but the nurse takes care of a comatose patient lying in bed for weeks and months, putting in a lot of effort but is not clearly shown in the payment price list, causing Nursing income is often low, undervalued.

Mr. Mu also expressed his desire to increase the number and quality of doctors, using more nursing experts to train nurses, instead of having many doctors teach nursing as at present. “We are a rare country in the world that uses a lot of doctors to teach nursing, this is like using the army to teach the police, we can shoot very well but our investigation skills will be limited,” said Mr. Mu. said and emphasized “nursing is a practical profession, requiring practical knowledge”.
